Island Occupational Medical Resources, P.C. (IOMR) was established in 1995, by Jorge L. Gardyn, M.D., F.A.C.P., to provide professional medical services to examine firefighters according to NFPA, OSHA, PESH, and DOT regulations.


IOMR’s commitment is not limited to the annual physical examinations, but is aimed at prevention of health related incidents as well as to support your medical needs throughout the year.  Immunizations, Return-to-Duty evaluation and Worker’s Compensation cases, including post-exposure prophylaxis for Blood Borne pathogens, are handled at our facility.


IOMR employs professional medical staff to perform all types of occupational medical examinations. Our medical staff is DOT certified and registered by the Federal Motor Carrier Safety Administration (FMCSA), and New York State licensed, registered and/or certified as Urine Drug Collectors and Breath Alcohol Technicians.

Firefighter examinations are available at your facility on weekends, and/or evenings.  Your firefighters may also be examined in our office in Amityville at their convenience.  The fee for full examination remains the same, regardless of where the physical exam is performed.

Scope of Service

Annual & Pre-employment physical examinations may consist of:

  • Medical History

  • Vital Signs: Height, Weight, Blood Pressure, Pulse, Respiration, BMI

  • Physical Examination

  • Pulmonary Function Test

  • EKG-12 Lead Electrocardiogram

  • Complete Blood Count, Blood Chemistry, Lipid Profile

  • Audiogram

  • Visual Acuity and Peripheral Field Testing

  • Fitness-for-Duty Statement issued to the Fire Department

Additional tests that may be required as part of the exam: 

  • Hepatitis B Screen and Vaccination, if applicable

  • PPD (Mantoux TB Testing)

  • Urine Drug Screen including MRO Services

  • Mask Fit Testing

  • PSA Test

  • Chest X-ray: baseline and every 5 years as required by NFPA 1582

  • Cardiovascular Screening

  • Rubella, Rubeola, Mumps and Varicella Titer & Vaccination, if applicable
